  • Date and Time of Event

  • Specific Location Photos and Videos Were Taken From

  • Approximate Wind Speed and Direction

  • Identification of Where Dust Originated From (if possible)

  • Direction That Dust Was Blowing

  • Note if Dust Was Blowing Off Property
